Credit Card Authorization Form
A credit card authorization form may be requested to be completed, signed and returned by a cardholder who grants Footsupplystore.com permission to charge their credit card. This may be requested by Footsupplystore.com for an order(s) taken over the telephone. If requested by Footsupplystore.com, the form must be completed can either be faxed back to us (973-465-0822) or emailed to us at sales@footsupplystore.com prior to the product being shipped. This form, once completed and returned by the credit cardholder, gives Footsupplystore.com the authority to charge the credit card for the order and amount requested by the cardholder for a specified order as listed on the credit card authorization form.
